Health Partners of Western Ohio receives nearly $100,000 grant

LIMA — Health Partners of Western Ohio received a grant of nearly $100,000 in response to its recent improvements, according to a press release.

The U.S. Department of Health & Human Services awarded $97,367 to expand quality improvement systems and infrastructure as well as improving primary care service delivery.

Health Partners, formed in 2003, is a federally qualified health center, operates 12 sites in Lima and Kenton locally and Bryan, Defiance, New Carlisle and Tiffin outside the region. It provides mental, dental, behavioral health, clinical pharmacy, dispensing pharmacy, substance abuse treatment, chiropractic and social services.

The group received a quality award for Health Center Quality Leader, achieving the best overall clinical performance among all health centers.

The federal government also recognized it as an Access Enhancer, as the total number of patients served and receiving comprehensive services increased between 2014 and 2015. It also received Patient Center Medical Home Recognition for each of its delivery sites.

“They reach beyond the walls of conventional medicine to address the factors that may cause sickness, such as a lack of nutrition, mental illness, homelessness and addiction,” according to a press release from Health Partners.
