Lima Memorial Home Health Care works with patients, families, caregivers and physicians to help people maintain their independence at home after being discharged from the hospital or a long-term care facility. At Lima Memorial Health System, we believe some of the best medicine a patient can receive is having the ability to recover in their own home. If a hospitalization is not necessary, LMHS Home Health Care can provide the care needed in the comfort of one’s home.
Our staff will be there to provide the medical services needed while teaching patients how to continue their own treatments when the time is right. All treatments are under the direction of an ordering physician. Caregivers and the patient are included in the development of the plan of care as well.
Preparation for home care begins long before the staff arrives at a patient’s home. Our staff works with the physicians, other healthcare providers and insurances to coordinate the aspects needed for in-home services.
Our team of highly skilled disciplines includes Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work and Home Health Aide.

We provide services in a 50-mile radius of LMHS, including in Allen, Auglaize, Hardin, Logan, Mercer, Putnam, Shelby and Van Wert counties.
Home Health Care, provided by Lima Memorial Health System, provides a wide range of health care services: Skilled nursing, patient assessments, patient/caregiver education, wound care, IV therapy, diabetes self-management education, well-mom/well-baby assessments, infant/pediatric care, ostomy care, blood drawing and specimen collection, nasal and abdominal tube feedings, medication and disease process management education, physical therapy, occupational therapy, speech therapy, medical social worker, home health aide for personal care, and dietitian consultations.
Anyone, regardless of age or financial situation, can receive home health care. Services offered by LMHS are paid for by Medicare, Medicaid and many private insurance companies. If necessary, other arrangements for payment can be made.
