No citations in refinery death
A gasoline tank is seen at the Husky Refinery on the South side of Lima. On Nov. 5, Adrian John Flores, a contracted worker from International Tank Service, was killed during an accident while he was on a lift inside the structure. Refinery Officials said Flores was roughly 50 feet above the base of the 56-foot tank when he fell to his death.
