TOLEDO — Lutheran Memorial Home, a Lutheran Homes Society (LHS) campus located at 795 Bardshar Road in Sandusky, announces that it has earned a 4-star rating from the Centers for Medicare and Medicaid Services (CMS).
The CMS Five-Star Quality Rating System rates the nation’s skilled nursing facilities on a 1- to 5-star scale, based on the results of on-site surveys to determine compliance with federal and state regulations. Lutheran Memorial Home’s overall 4-star rating equals “above average,” and is based on the Health Inspection, Staffing and Quality Measures ratings combined into one.
Lutheran Memorial Home also earned a 4-star Health Inspection rating, which is based on the nursing homes’ health care survey results, and a 4-star Quality Measures rating, which is based on resident assessment data that shows how well the nursing home cares for residents’ needs.
“We were deficiency-free in infection control, dietary and nursing care during our annual state survey,” explained Executive Director Deanna Dothard. “We are now one of the highest rated skilled nursing facilities in the Sandusky area, and our entire team has worked extremely hard to make sure all of our processes and procedures ensure the highest quality of care for our residents,” she added.
