ST. MARYS — Grand Lake Physician Practice’s Primary Care and Pediatric practices recently received renewal of their National Committee for Quality Assurance Patient-Centered Medical Home Recognition.
Grand Lake received the PCMH Level 3 Status Practices, the highest achievement level, for using evidence-based patient- centered procedures that focused on exceptional care and long-term participative relationships.
“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane, in a news release. “Recognition shows that The Grand Lake Physician Practices have the tools, systems and resources to provide its patients with the right care, at the right time.”
