ST. HENRY -- The Occupational Safety and Health Administration has fined Tumbusch Construction in St. Henry nearly $54,000 in response to three citations issued in June when the contractor was working in Elida.

OSHA said inspectors at an excavation site found workers were not protected from cave-ins, had no means to escape the trench, and were not protected from overhead work and rigging equipment. OSHA issued one willful and two serious citations for the violations.

Inspectors found similar violations at a Tumbusch work site in January after an accident in Auglaize County that killed a 22-year-old Darke County man.
